That's normal practise here in France – you need a medical certificate to do anything at all sporty, even a 5k fun run, – unless you have a licence for that sport, in which case you'll have had a full medical each year as part of the licence application process. Because it's the norm, doctors are more used to it and it just counts as a normal doctor's visit – costs about 22 euros, and you get most or all of that back from the state and/or private medical insurance.

Could you have the medical in France using your E111?

Don't see why not - before I was fully in the French system I once went to casualty at the local hospital here on a saturday night and got a certificate from the duty doctor! Mind you, saturday nights are a bit less of a battle ground in rural France than in UK! You'd still have to pay the 22 euros, and then try to claim it back, but even if you didn't manage to get it back, you'd still be quids in. Bear in mind most sportives etc insist the med cert's issued in the month (sometimes three months) preceding the event so you'd have to a: make sure you knew which, and b: get your timing right.
