This may seem a stupid question to most, but one i've been curious about 10 speed double large chain ring & smallest gear on rear cassette is this 1st gear or 20th? (please feel free to write stupid comments below answer )

I would say that would be 20th gear...think like driving a car...1st gear for the steepest hills, top gear (20th) for going fast...but to be awkward you should think of a bike like a 4x4 in terms of gearing, A landrover has 10 forward gears but you class it as a 5 speed gearbox with high and low ratios, as the bike has a crossover of gear ratios just the same it doesn't actually have 20 gears, actually only 12 or 13 different ratios. When cycling I class it as high 6 or low 6 for example..easier to discuss what gear you use when boring the pants off other cyclists than saying 16th for high 6 or just 6 for low 6! Hope this has added some confusion to your life!

Most people I know refer to the gear by either teeth (eg. 48x16) or the inches it represents (eg. 82"). And "top" and "bottom" for the extremes.

That's top. Small ring/large sprocket is bottom. Putting numbers on them doesn't really help since there isn't a natural progression from 1st to 20th, because there's a big overlap between ratios on the two chainrings. The exact overlap depends on chanring size and cassette ratios. SO long as you're in the right gear, it doesn;t really matter what you call it!
